REFERRAL

Referring health professionals can make a referral by using the referral form.

​Referral form can be faxed to: 02 9477 1672, or email the form to us. 

​Once the form is received, Dr Chung or Dr Lim will triage the patients and an appointment will be allocated accordingly. 
Patient will receive a phone and/or letter of confirmation detailing the appointment.
